The pages in this site were created and designed to be an informative and interactive way for us to communicate with you, the public that we serve. The information provided by this site is broken down into categories listed in the menu which appears on the left.   • Forms - Downloadable forms often requested, in .pdf format. These require Adobe® Reader®, available as a free download. Forms presently available are:
      • Business Registration Form. Secure your business by giving the Bernardsville police the information needed to contact you.
      • Hunting Permit Application Form. You must get permission from each landowner on a separate copy of this form before filing it with the police--land hunted must be 5 acres or more per owner.
      • Motor Vehicle Accident Report Form. Must be filed with state if you are driver of a vehicle involved in an accident resulting in injury to or death of any person or $500 or more property damage, unless police file a report. We suggest you print this state-designed form on two sheets of legal-size paper. If you print it on both sides of one sheet, the arrows in Section B will not line up with the correct boxes. If your printer, software or paper cannot handle legal size, please pick up a hard-copy (paper) form at Police Headquarters.
